The Therapy Model

Return to work is part of the therapeutic contract — from session one.

BridgeMed Health's in-house therapy team delivers structured, RTW-anchored psychotherapy using CBT, ACT, DBT, Behavioural Activation, and Motivational Enhancement. Return to work is part of the therapeutic contract from session one — not an afterthought.

Progress reports use function-focused language — Activities of Daily Life, workplace task capacity, preserved abilities — so case managers have the clinical information they need to plan a Gradual Return to Work (GRTW).

How Therapy Unfolds

A three-phase structure, designed for closure.

Phase 1 No charge if FPA referral

Initial File Review

  • Medical history review
  • Confirms appropriateness for RTW therapy
  • No charge if referral is based on a BridgeMed Health FPA
Phase 2

Initial Session

  • Shared goal-setting and motivational enhancement
  • Confirm employment to return to, and accommodations needed
  • Confirm with family physician — no contraindications
Phase 3 8–10 sessions average

Ongoing Sessions

  • CBT, ACT, DBT, Behavioural Activation, and Motivational Enhancement — all RTW-anchored
  • Standard reporting at sessions 4, 8, and final
  • 1–2 sessions strategically held post-RTW to consolidate gains
  • Final report summarizes functional gains and accommodations
Reporting to Case Managers

Visibility at every milestone.

Case managers receive structured, function-focused reports throughout — no surprises, no gaps. Every report is written in plain language case managers can use for accommodation planning and GRTW coordination.

Report Timing What It Includes
Initial Session Report After session 1 Therapy plan, session estimate, employment to return to, initial accommodation dialogue.
Every-4th-Session Report Sessions 4, 8 Function-focused RTW assessment — Activities of Daily Life, workplace tasks, preserved abilities; initial GRTW plan shared with case manager for joint planning; medication comments.
Final Report At closure Functional gains summary, RTW status, workplace accommodations, file closure statement.

Why these milestones?

Reports at sessions 4 and 8 are deliberately timed to surface the initial Gradual Return to Work conversation early — before the case stalls — and to give case managers a structured input point for accommodations. Average closure is 9.6 sessions, so a 4-then-8-then-final cadence captures the full arc of treatment.
